All Upward Coaches and Referees must complete an application and submit to a criminal background check.  Enlisted coaches, assistant coaches, and referees may print out the following forms and fill them out at your convenience.  After completing the forms, please return them to Jana or Laura  at First Baptist Church so that we can complete the process.
